The role of the Streaming CNM is to facilitate a safe streamlined pathways of care for patients from the emergency department and community settings who require acute medical assessment.
They will ensure that multidisciplinary and multi-agency inputs are synchronised and targeted to match individual (acute and post-acute) care needs.
